Submit a Complaint Online

Please complete the below information to submit an online complaint to the College.  We will need the name of the occupational therapist you are concerned about, as much detail as possible about the nature of your concerns, as well as your full contact information.  If you do not know the name of the occupational therapist, please review the online register here or contact us for assistance. 

Additional information or supporting documents that you wish the College to consider may be uploaded with this complaint form.  You should be aware that a copy of the complaint and any supporting documentation may be shared with the occupational therapist during the course of the investigation.

If you have a complaint about more than one occupational therapist, you need to submit a form for each occupational therapist.
